. How to Use Forms  

 

 

   

Cooke's Law forms are meant to be accessible and functional.

However, forms offered on this site are not state specific (unless otherwise stated) and Mr. Cooke makes no representations, warranties or guarantees that any forms are valid for your particular purposes in your location.

To use Cooke's Law on-line forms, you must have Adobe Acrobat Reader™ on your computer. It is available for download and installation at "http://www.adobe.com".

When you click on the selected form, your browser should launch Adobe Acrobat Reader™ automatically.

Use Adobe Acrobat Reader™ to view and fill in the form.

When the form appears on your desktop, hit your "tab" key to find the first form field. When you place the hand over the cursor, a box will appear that tells you the information needed to fill-in the empty field. Tab from field to field filling out each as you proceed. You can also click on obvious field spaces or on radio buttons, check boxes, etc. to show your answers. Note that the fillable fields are equipped with automatic line-wrap and shrink to fit features. If you type more characters than fit on one line, the text will automatically wrap to the next line. When you move to another field, the text that you just typed will automatically shrink to fit the field.

When you have finished, you can print the form. When you are done, exit Adobe Acrobat Reader™ to return to your browser. For most of the forms, you will not be able to save the completed form to your local computer.
